| A-0103 | Sleep Disorder | Pediatric obstructive sleep apnea (POSA) is a common yet often underdiagnosed sleep-related breathing disorder in young children. We report the case of a 4-year-old male who was brought to the otolaryngology clinic by his parents with concerns of habitual snoring and abnormal sleep behavior. The child exhibited multiple sleep-related symptoms, including prone sleeping position, frequent talking or vocalization during sleep, drooling, loud snoring. During wakefulness, he was noted to have persistent mouth opening and predominantly mouth breathing, which became more pronounced during physical activity. Otolaryngologic examination revealed significant adenotonsillar hypertrophy without evidence of craniofacial abnormalities. Overnight polysomnography was performed to further evaluate the symptoms and demonstrated an apnea?hypopnea index (AHI) of 8 events per hour, with a minimum oxygen saturation of 85%. Based on the clinical presentation and sleep study findings, a diagnosis of mild to moderate pediatric obstructive sleep apnea was established. The patient subsequently underwent adenotonsillectomy. Postoperatively, the parents reported complete resolution of snoring and sleep-related symptoms, along with marked improvement in sleep quality and daytime activity.
